In the report, it said that the main cause of the accident was the inability of pilots to notice and arrest the lateral drift of the chopper to its left due to lack of ground reference until the main rotors came into contact. [break]
Commission´s Chairman Nagendra Prasad Ghimire submitted the report to Minister for Tourism and Civil Aviation Sharat Singh Bhandari.
The report said that the other cause of the accident was due to lack of risk assessment and mission planning prior to the flight by the company, inadequate reconnaissance and improper judgment regarding selection of landing site.
Similarly, other causes are inadequate crew training standards, training procedures for the type of mission undertaken and lack of crew roster policy in the company in relation to the nature and level of mission difficulty.
Releasing the report, Bhandari said the recommendations made by several commissions after plane and helicopter crash were not implemented till the date. Thus, a commission formed to implement all those recommendations is working, he said.
There should be adequate security and safety measures in the civil aviation sector for the promotion of tourism although it is a challenge, Bhandari said. Safety training should be provided and the Tourism Ministry is serious about it, he said.
The commission recommended that current FOR regarding high altitude training, two crew-member requirements in high altitude missions, current pilot licensing policy should be reviewed and revised.
Other recommendations includes the flight operation division of Aviation Safety Department should be appropriately manned to oversee helicopter operations.
Commission members Suresh Acharya, Deepak Raj Koirala and Capt. Pramod Kumar Lama presented the study report.
A five-member investigation team was formed on Nov. 9 and its started its investigation on Nov. 10.
The AS350B3 ´Ecureuil´ helicopter owned and operated by Fishtail Air crashed when it was on a rescue mission on the North ridge of Mt. Amadablam at an elevation of 6,350 meters.
