However, if you’ve misplaced yours, then here’s what you should do to get a copy of your Covid-19 immunization card according to the National Health Education Information and Communication Centre:
- If you have already had your first and second Covid-19 immunization shots, you can obtain a copy from the vaccination center where you received your second dose; or the Health Center Department of the municipality where your vaccination center is located.
- If you received a Johnson & Johnson immunization shot, go to the vaccination center where you received it; or the Health Center Department of the municipality where your vaccination center is located.
- If you only had one Covid-19 immunization shot, obtain a copy from the vaccination center where you received it or the Health Center Department of the municipality where your vaccination center is located.
