The ambulance was carrying Kripal Mahato, 52 of Mahottari, who was referred to Kathmandu from Dharan. Mahato died on the spot.
Those who were injured in the incident are Bharoshi Mahato, Laxman Mahato, Rameshwor Mahato and driver Shyam Paudel.
The injured are being treated at the Sub-Regional Hospital in Birgunj.
